Student Information Form

This form should be completed and turned in to your child's teacher. The information would be used in the event of an emergency or unscheduled school closing. There is also a section that is used to give permission for your child to be videotaped, interviewed, or photographed for release within the Howard County School System. Remember if this information changes it is your responsibility to contact the school with the updates.
